Medication management refers to the ongoing, structured process of ensuring that a patient's medications are appropriate, effective, safe, and used correctly to achieve the intended health outcomes. It goes beyond simply prescribing or dispensing a drug — it encompasses how medicines are selected, prescribed, administered, monitored, and reviewed over time to optimize their contribution to a patient's care. 

Core components. For an individual patient, medication management typically includes: 

Appropriateness review — confirming each medication has a valid indication, is the right choice for the patient's clinical status and comorbidities, and avoiding unwarranted poly-pharmacy or redundant/unnecessary drugs.

Effectiveness assessment — checking that doses are adequate and the intended therapeutic response is being achieved, with dose titration as clinically indicated.

 

Safety monitoring — evaluating for adverse drug effects, drug–drug interactions, supra-therapeutic doses, and relevant labs/vital signs.
 

Adherence support — assessing whether the patient can and does take medications as prescribed, and addressing barriers (cost, complexity, understanding).

Patient education — ensuring the patient understands their treatment plan, how to take each medication, and why adherence matters (e.g., using teach-back).

 

Documentation and follow-up — maintaining an accurate medication list, reconciliation across care transitions, and scheduled reassessment.
